Admission Criteria

To be admitted to the Polk County DAC, there must be a referral through Polk County Social Services.  If you need to contact them call 218-281-3127 and ask for the intake worker.  Once that referral is made, they will determine eligiblity.  To attend the DAC, you must be at least 18 years of age.  There must be a diagnosis of a developmental disability or a related condition such as Tramatic Brain Injury (TBI) Cerebral Palsy (CP) or a dual diagnosis of Mental Retardation or Mental Illness.  You can contact us directly at 218-281-4181.  We look forward to serving you!
